Care Management Team
Working Age Adults
Our client, a mental health trust in the Yorkshire area, is seeking a qualified and experienced Social Worker for a temporary contract. The team is a care management team who deal with working age adults suffering from non-psychotic disorders.
The post holder will form a key member of a multidisciplinary team, taking responsibility for assessments and the care management and care coordination of a caseload of clients. You will c! arry a complex and varied caseload, delivering care inline wit! h the Care Programme Approach.
You would work in collaboration with other colleagues, ensuring a multidisciplinary approach to intervention, support families and carers where needed and implement safeguarding policies if appropriate
